Need a good user profile survey/questionnaire for website prototyping

Hello,

We are getting ready to prototype a new banking user interface and we would like to collect information on the potential users via a survey/questionnaire (i.e. how often do you use online banking, how familiar are you with the internet, etc.)  Can someone please point me in the right direction as far as a sample survey or good questions to ask?
