If those do not help can you tell me if this is a relatively new install?  Can you recall anything that changed when this started occuring or has this always been an issue?

What is the history of this server?
Clean build, upgrade?
What about the org? Clean to Exchange 2003, upgraded from a previous version via swing?
Is the server on the latest service packs?
Is the closest domain controller also a global catalog? Is the domain controller behaving itself?
Are the Exchange services running under a specific account, or the local system account?
