Multiple Bridgehead servers?

I have one bridgehead in my HQ site, but was wondering if I need more.  Should I have a bridgehead at every site? and 2 bridgeheads at large sites with multiple DC's?  Replication has been a problem lately and I am trying to speed up and organize replication.  Also, should the exchange server be a bridgehead for SMTP?
