Trouble Publishing Terminal Server in Standard ISA 2004

The goal is to publish 6 terminal servers (TS) via standard ISA 2004. 5 of the 6 have been successfully published.  It is the 1 remaining TS which I write.  Here is the rules configuration.

Name:  RDP ICMTERMSRVR03:1399
Action:  Allow
Traffic:  RDP (Terminal Services) Server
   Firewall Port: 1399
   Publish Server Port:  3389
   Source Ports:  Allow any port
From:  Anywhere
To:  Internal IP Address
  Forwarding:  Request appear to come from ISA Server
Networks:  External / Local Host

There are 6 rules publish using the above configuration.  The Firewall Port is only difference between each rule, because each TS is assigned a different firewall port.  Both Windows 2003 and Windows 2000 TS are being published.  It is a Windows 2000 TS which does not work.

Monitoring shows the request for that requests for failed TS publishing rule are coming through the firewall.

This TS was first assigned firewall port 9300. When switching the port with a working published TS, the working published TS failed. So I thought it was the port.  So changed firewall port from 9300 to 7123, which did not resolve this issue.

Have assigned the failed TS ip address to a working TS publishing rule.  It worked.
Have assigned a working TS ip address to the failed TS publishing rule.  It failed.

All TS can be connected to from the ISA server (local host network) using the RDP client.

Is there a limit of 5 TS publishing rules?
