What is the worst sysadmin mistake you have ever made?

We have all deleted a file or rebooted a server at the wrong time.  Please let us have your horror stories - how many users did you impact, did the company lose money, did you [almost] get fired, how long did it take to recover the service, and any other interesting war story details like how you battled to get it back after you broke it.

I'm deliberately assigning low points because I don't want people to try to outdo one another with my-incident-was-worse-than-your-incident type stories just for the points.  This is just for fun and reminiscing :-)

We recovered from a backup that was a couple of weeks old, so it was OK - but painful.

as they say "I'll never do that again!"

Ouch!
This one wasnt me, but a guy i worked with for a while:

Went to backup Mailbox store on Exchange. Started to backup. Finished backing up. "Oh.. where has all my mail gone... Oh.. Where has ALL the mail gone?!" turns out he forgot to stop the exchange mail store process. He corrupted 1200 users mailboxes and because there was no backup of it, he couldnt restore it.

Ahhh... memories...
