Creating a form with tick box and text that changes colour

I want to have a word document with a form which has a series of tick boxes (or preferably radio buttons if word can do that).

What i want is that each radio button or tick box has some text associated with it.
If you tick the box i want the associated text to change from black to BOLD RED so that it really stands out.
If you remove the tick then it should revert to normal black text.

There will be about 15 of these tick box + text items on the form.
