HP Smart Array - Logical disk failure
HW Make: HP DL380 with Smart Array 5i
OS: SBS2003
Problem: Something happened overnight and the OS would not boot. I was getting an error message about a logical disk being disabled. It gave me two options F1 to leave the current state of logical drive as disabled. F2 to enable logical disk but risk data loss. After choosing F1 it would give the error non system disk. F2 let me get into the OS where it started fixing errors on the drives (took an hour) But now it gives me the error about SAM not initialized obviously seems that AD is corrupted. I tried the NSDUTIL but it cannot connect to the AD database. What is your take on this?

It is quite possible you have bad stripes in your harddisk array. Take the advice above, run chkdsk first. Then use the smart array tool to capture the controller status then check if there is any bad stripe recorded. It yes, you may need to go for steps to clear the bad stripe table.

Thanks. When I managed to boot into Windows it started CHKDSK automatically and started deleting and adding indexes to files. In other words, it started repairing the drives. After it was all done, and I rebooted the server, the active directory error pop up shows the following:
lsass.exe-System Error: Security Accounts Manager initialization failed because of the following error: Directory Service cannot start. Error Status: 0xc00002e1. Please click OK to shutdown this system and reboot into Directory Services Restore Mode, check the event log for more detailed information.
I have already read through the following article

What I now want to find out is how it happened and what steps should I take to restore the system. Do you think it was a hd failure? if that is so then I need to change the HDs and build a fresh server then restore files from backup. Any advice would be appreciated.

The HDD is RAID using the smart array 5i controller. I will contact Microsoft today if solution not found. Many thanks

My understanding that it is RAID5 but I will confirm this with the person onsite. Thanks

Doesn't sound like it was caused by a hard disk failure, more likely a controller failure. The ADU will tell for sure but needs Windows working first, http://h18023.www1.hp.com/support/files/server/us/download/26010.html Reinstalling or repairing Windows won't affect it since it reads the SMART data off the disks.

Thanks I have used the Smart Start CD for this which has the ADU and ACU The ACU shows a red alert on the logical disk which says that there is a logical disk failure and data has been lost. Doesn't look great.

I replaced all the hard drives and it worked fine for 2 weeks but the same problem has resurfaced and the server cannot boot. Something about logical drives. I suspect that this is a controller issue which is an embedded Smart Array 5i. Any ideas? If it is embedded that means the motherboard needs replacing?
It's imbedded in a clip similar to a DIMM socket, near the PCI riser. There may be an associated battery at the front right next to the CD/floppy.

It is implied that you get a smart array controller rather than some other type that will not understand what is on the current disks of course. A 5302 , 6402 or 641 etc., not a Dell perc or IBM serveraid.
You might have to boot from CD.
