We have a problem receiving emails from a specific user/domain.  The return that  this user gets says that the email to us has been delayed and the delay reason is:  SMTP error from remote mailer after initial connection: host {host name}  {ip address}: 421 Temporarily unavailable service due to potential threat.

Some of this persons emails have got through however.  We are running exchange 2003 with McAfee Webshield box providing spam filtering

It is not an Exchange message - so if the McAfee product is scanning email then you need to look at that product. See if it has some kind of white listing feature.

If it continues to cause problems then it is not fit for purpose and should be removed. If the email messages flow correctly, then you know where the problem is.

It was the Mcafee box. I found the reputation filter that would drop the connection if it didn't like the IP address, I have now turned it off
