Looking for a Survey build with ColdFusion, and Access

Hi
Does anyone know where I can find a survey (about 10 questions) build with ColdFusion, Dreamweaver and Access as database.

SidFishes, this one is ok and works well, but it uses email or writes a csv.  Any chance of finding one that talks to an Access database, so info is saved immediately and also includes a bar to monitor progress.
