Sent emails not ending up in sent mail folder with Outlook2003 on a Hosted Exchange Server

Working with a hosted exchange server, and outlook 2003 using cached exchange mode,  a user was having problems with some sent emails not appearing in the sent mail folder,  sometimes these emails would end up in a special conflicted file folder, some would just seem to disappear.   This was only happening on one users computer,  I tried to solve this by repairing the install of Microsoft Outlook 2003, and then by removing Outlook 2003 altogether and reinstalling it, this did not fix the problem.  Recently this problem started up on a second users brand new computer.
Does anyone have idea what could be causing this.

That usually means the sync process has failed - network connectivity or something like that.
Recreating the Outlook profile will often fix the problem - reinstalling Outlook will usually not fix these types of problems.
