Entering and displaying (using PHP and MySQL) predefined date and time slots on a web form

I use PHP and MySQL and I need to set a registration web form where users can choose a schedue from pre-defined dates and times. There are 4 dates and 16 time slots for each date. Users would have to enter their personal details first and then select the day and time that suits them and submit it (into the MySQL database).

My question is, what is the best way of structuring the database and entering these predefined dates and times and then display them on the web form. I would also like to hide the date and time slots that have already been taken from the web form so new registrants do not see them.
