Check Control Panel, Sounds and Audio Devices.  On the Volume tab, make sure "Place volume tab in taskbar" is checked.
XP:
Start -> Control Panel -> Sounds and Audio Devices -> Check "Place volume icon in the task bar"

Vista:
Right Click System Tray -> Properties -> Check "Volume"

Note: The first article contains steps which is the same as what others posted. The 2nd link explains what to do if you receive an error message when adding the volume icon.
Glad that I could provide something useful.

Thank you for the 'A' grade.
