skip logic for radio buttons using a questionaire format

I have a questionnaire/survey with about 10 questions that consist of mostly radio buttons. Each question has 3 or more choices. Ex. Yes, no, dont know or a scale from 1 to 5(not at all to fully used). Some questions say skip the rest of the section if answered not at all. I dont know if I should display (or hide) the rest of the questionnaire or not? And if yes display this question. However if the user revisits the questionnaire they should have the option to change their answers. All responses are submitted to an SQL database using coldfusion. I need code that I can reuse and update! Should I disable or hide all questions but the first question? Please help me, I need detailed code and examples!

That is ok but I decided that I don't want to layout the entire questionaire in advance.
