Can we set a form in outlook and update mails to excel

Hi,

Is there a macro or a way to send a mail to users to update there details.
Headers and boxes for user data to be entered.

Ex:
Name    Age   Computername   Place   City

And boxes down for the user to enter.And a ok button to send the mail once done and a reset button.Once the mail comes back to me it has to be updated automatically to a mentioned excel.
