I have seen this happen with antivirus applications that have held on to the messages and only release them when the server is rebooted.
It could also be the greylisting bug.

Using Exchange 2003 SP1. And busy excluding Exchange files from Antivurus.
Any reason you are not using Exchange 2003 SP2? It is rock solid and your are basically using a product that is almost two years old.

Although Exchange 2003 SP1 is mean't to fix the badmail issue I'm not sure what version you are talking about. Each time I've seen this happen it was due to the old e-mail in the badmail folder being re-submitted after the reboot. I solved the issue by routinely cleaning up the badmail folder using this tool.
