Outlook fails to open PDF attachments randomly

I have an Exchange user who when recieving forwarded email that have .pdf attachments within them, sometimes the PDF opens if she opens the attachment from Outlook, and other times the following error comes up: "Can't create file: image.pdf. Right-click the folder you want to create the file in, and then click Properties on the shortcut menu to check your permissions for the folder."

If the user clicks through some email, and then goes back to the email with the problem PDF, sometimes it work again randomly, and other times it simply won't work still. Very odd and annoying.

Off the top of my head, sounds like an issue with the permission of the local profile and permissions related to local settings folder.  Try deleting the Local settings folder.
Also try,

1. Open REGEDIT.EXE and go to Edit -> Find... In the Find dialog box type "OutlookSecureTempFolder" without the quotes and locate that registry key.

2. That key will contain the actual folder location, and will look like:

       C:\Documents and Settings\%USER_NAME%\Local Settings\Temporary Internet Files\OLK#\ (where # is a random letter or number)

3. Copy the location of that folder.

 4. Navigate to the file location. Delete the folder.
