I added the IM_DISABLED=1 line and it worked the first time I opened Lotus Notes, but then Messenger popped up again and when I look in notes.ini, there is a new line there IM_DISABLED=0. How can I get rid of it for good?

Did you try to remove the  Sametime Server in the Location Document as suggested in the link.

Set the location doc to NOT log on to instant messaging, remove the Sametime server from the location doc, and add the INI variable back in.  That should do ya.
