Templates in Certification Authority showing Element not Found

We are having a problem with our Certification Authority. It is running on Windows 2003 Ent x64 SP2.
When you click on the Templates node it says "Template information could not be loaded: Element not Found".
I tried issuing a certificate from our webserver and it is sitting in the Failed Requests node with the following errors:

Request status code: Element not found. 0x80070490 (Win32: 1168)
Request Disposition Message: Denied by Policy Module 0x80070490, Certificate Services could not find required Active Directory information

We need to get this working asap. Can anyone help???

I'm having a very similar problem, Am trying to add a custom template, and it's not showing up in Certificate Templates, and so when trying to request a new cert from Cisco ACS i get the same error you do:

Event Type:      Warning
Event Source:      CertSvc
Event Category:      None

Description:
Certificate Services denied request 10 because The request contains no certificate template information. 0x80094801 (-2146875391).  The request was for S=xxx C=xx, O=xxxxx, CN=xxxxxxxxx.  Additional information: Denied by Policy Module  0x80094801, The request does not contain a certificate template extension or the CertificateTemplate request attribute.

Anyone have any answers? I'll update if I find anything myself.

I ended up calling Microsoft on this. We had to use adsiedit to manually re-enter the ca certificate. I don't know the exact steps and I would NOT recommend that this be done without a Microsoft tech walking you through each step. They said if the cert re-install didn't work then the solution is to re-install CA on the server.
