this means that the email address that you have assigned to the new user i already in use by another user/object in exchange

are you trying to assign an email address manually to the new user?
if yes, then exchange should do that for you. don't assign the email address manually

What does it mean to be "Always On"?

Is your cloud always on? With an Always On cloud you won't have to worry about downtime for maintenance or software application code updates, ensuring that your bottom line isn't affected.

but when I send a test mail to john@my.com , it returns "Your message did not reach some or all of the intended recipients." i can't seem to figure out what's going on... maybe john@my.com is stuck somewhere?

after you delete an email address or an object you need to wait for some time for the changes to replicate

ok.
do you have the option to recycle the information store service?
it still appears that the deletion has not replicate within exchange, that is the reason it is assigning user2@domain.com

if you restart the information store service, all the users on this server will lose their connection to the server
to restart the service to the control panel and open services
you would find microsfot exchange information store service there
