MS Access Query parameters in form for report

I need a report that has 2 parameters: division and type.  I want to create a form that lists all possible division and type accounts in a combo-box so that the user can easily select a value.  For whatever the user selects, I want it to populate the "Type" report.  How can I do this?  I created a query that forces me to type in the division and type.  I also have created a form that lists all possibilities but I cannot get it to go to the report.  When I open the report it still prompts me to type in the division and the type.  How can I get the form to talk to query to populate the report.  Any suggestions?  I would like the form to pop-up when the report is selected and then once the user selects both the division and type maybe they can press a button to generate the report.

You have to open the form first and then run the report.
Using Form controls in the query criteria will not force the form to open.
