Error on form submission - using pre made form

I am trying to get my form working. I am using my tech tite form, pre made.

I have tried looking through the docs, but thought I would give the PHP experts here a chance to see if they find the problem.

I am getting:

An error occurred while processing the form .

Our staff have been alerted to the error .
We apologize for any inconvenience this error may have caused.

Any suggestions on what I can do? You can fill out the form but you only need the fields email address and first name. You dont need to fill out the whole thing.

The error message is not very forthcoming. You need to add some extra code to give you more of an idea of what is going on.

Round about line 4813, do this

....
....
