Need more info.

What is the outbound rule applied to ISA for internal to external web traffic?
What authentication has been applied? All users? Authenticated users? AD group?
Are you using the ISA firewall client?

Have you made a boo-boo and ticked the all users must authenticate box in the configuration-networks-internal - properties section?
Any update?
