Do you have UPDATE or DELETE statements in the form/block?
Possibly your Form experience  deadlock.

If it was a deadlock, would that not show up in the alert.log?  There are no error messages in the alert.log

foms version is 6i database oracle version 9.2.0.7

I am not sure which form is actually causing the errors, we are currently trying to narrow that down

I do not think that this will be registered in alert.log only. See also Application log.
Also see for Network errors on listener.log

Use the SQL*Net trace file to look for any ORA-XXXXX or TNS-XXXXX errors
that may actually be the underlying cause of the FRM-40655 error reported
    by Oracle Forms.

The SQL*Net trace file will allow you to:

o see the exact text of every SQL statement that the client is sending to
      the server
o see the response that the server sends back to the client
    o see malformed SQL statements
o see the errors that the server returns

Review the sqlnet.log file for any TNS-XXXXX errors that may have occurred
that may actually be the underlying cause of the FRM-40655 error reported
    by Oracle Forms.
