Writting an I.T Maintenance Contract

Hey,
I am looking at offering 'maintenance contracts' as a service. This will involve a set monthly fee for us to manage a companys I.T systems.

I am looking for some examples of contracts that I can use that outline what is included in the monthly fees. I dont know where to start or how to write such a document.

Any suggestions would be much appreciated.

Many IT Consultants are moving towards a "Managed Services" environment rather than break/fix.  

Many of the Managed Service Provider Services vendors offer lots of sample agreements, etc.  I'd suggest that you start by looking at http://blogs.msdn.com/mssmallbiz/archive/2005/10/25/484753.aspx which has a good list of resources.
