Control Box won't appear when creating an Outlook form.

I'm using Outlook 2007 and when trying to create a form, I can't get the Control Box to appear. I've tried all four of the different methods described in the help files and nothing happens when I do them. Also, I can't even seem to find out how to type in a text title on the top of the form.

Well, I seemed to have solved the problem. I don't know if this is a bug or not but the Control Tool Box won't always appear if the Field Chooser window is open. This doesn't always happen but if the Control Toolbox doesn't appear and I close the Field Chooser then when I click on the Control Toolbox it will appear.
