Mail this (current) page to a friend

Hi,

I need a code (php or html) who has a button on every page of my site. When you click on that button (mail this page to a friend) it should get a form with some fields like:
Name to
Email to
Name from
email from

It should tell in the body that you have to visit the page.

www.url.com/page1.php
or
www.url.com/page2.php
etc etc
