How do you determine the last time an online defrag occurred on all information stores? or offline defrag?

Offline is not possible. You shouldn't consider offline defrag something that needs to be done on a regular basis.
Online defrag is recorded by event ID 1221 in the event viewer of the server. You may seen it multiple times during the hight - for each store.

What do you mean, offline is not possible.  Offline defrag does not show up in the Event IDs?

Offline defrag doesn't show up because it is not a system event. You have to physical stop Exchange and then run a tool on the databases. There is nothing to log.

Offline defrag is not a regular event and shouldn't be considered so. Many people like to treat as such, but it is something that should be hardly done, if at all.
