Sounds like you need to reinstall adminpak.msi to correct the issue. It can be found in the i386 folder on the windows server cd.

MAybe this will help

Problem: MMC crashes when started. When you start the MMC, it may die and either generate a Doctor Watson error or simply fail with no errors. This failure at startup often occurs when there are .tmp files left from a previous session.

Solution: MMC startup crashes are normally related to leftover .tmp files on your system. Clear all .tmp files from the Temp directory and restart the MMC. If the MMC still crashes,

Shut down user applications on the system.

Log off the system.

Log back onto the system.

Search for for *.tmp files in the c:\temp directory (the directory to which the TEMP variable is pointing).

Delete the *.tmp files.

Restart MMC.
Problem: MMC does not work correctly. You may find that the MMC behaves strangely, or it may not work at all. Problems usually occur when the system running the ISM (and any MMC-based snap-ins) has less than 2 MB of available disk space.

Solution: To resolve the problem, free up disk space on the system.

There are no temp files and the KB article doesnt seem to relate to my problem either.
The recommended fix to a problematic MMC console is to update the console: MMC 3.1 is available for download and I think it is a part of the adminpak. Try to update MMC.

http://support.microsoft.com/kb/907265
I found wadminpak.msi on the cd but no adminpak.msi...which one is the right one?  I am using Server 2003 R2 if thaty makes a difference.
I tried the KB article and it will not reinstall the MMC.  It fails with the message that I have a newer version. The wadminpak.msi also did not work.

The file location that is failing is:
c:\windows\system32\dsa.msc
I am not sure, but I think the admin pack is for SP1 and carries over to SP2. You may want to uninstall the admin pack and reinstall it. That might resolve the issue. If this is your role holder domain controller, you may wish to switch the role holder before uninstalling and reinstalling the admin pack.

So, you may be up to the latest version of MMC with a corrupt console. Reinstalling may be the only alternative.

You might want to look at this posting from one of our own.

Tried uninstalling and reinstalling the adminpack with no luck....
