Web Form Design - Best Practise

We want to design some forms with many form objects such as radio buttons, dropdowns etc.
We also want to design output tables with approx. 20 columns and some hundred rows.

1. How would you design a simple and good-looking web form?
2. How would you design a simple and good-looking table?

...are there any templates around (colors max. white, black, grey and red)?

2 Solutions
What I do is look at some high-end corporate sites, find one I like, view their source, and incorporate some of their design methods. This will save you a lot of time. Don't steal their proprietary images, but borrowing layout ideas is fair game.

I'd like to have a tool to 'paint' (pixel precision) all these objects on a plain white form sheet and later do some easy grouping and coloring. I've seen in Dreamweaver this could be done with Layers, but are there other methods around?
You need to use a nice clean set of CSS rules for the table so that all your colouring and formatting can be done from one source without editing the actual table.  Have a look at this site for tons of free examples of CSS tables... http://icant.co.uk/csstablegallery/
