How to make a form and email it to users to fill out and send back?
I'm looking to create a form in notes that will have items that need to be filled out like:

EDS Net ID:  
Primary Division  
Who is Participating?

Create a new Database, and put the form in there.

Send out an email with a link to the database, and a view that has the options that you want ( Create form )
but does not show any other forms, or simply put a button in the Email that opens a new docuement based on the form that needs to be filled out.
Make sure security is set to default autor, and create views so that olny authorized personnel can see the needed data.

Also check the 
for emplyee survey.

This is similar, and may save you work, since it is a customizable survey.

If you don't want your users to access the database directly, you could create a form that is sent in combination with the message to the user. You can also add the logic what should happen with that message when the user updated the form. You have to assign the form property "Store form in document" in the Form Properties box, or if you program sending the mails in LotusScript, you have to send the message using doc.Send(True).
