My Outlook rule does not work 100% of the time. It's supposed to flag all e-mails (green) from a specified sender (my boss).
I created rule in Outlook (back when I was using 2003) that whenever my boss sends me e-mail it will get flagged with the color green so that I won't forget about or miss them. Now some e-mails do not get flagged. I would open up the e-mail there would be a message saying "You made changes to another copy of this item. This is the most recent version. Click here to see other versions." 1. I never made any changes to that e-mail. 2. When I click to see the other version - that's the flagged e-mail. I don't know when this started happening. It used to be great when it was working 100%.

Have you tried deleting and re-creating the rule?
