Batch file to create and copy from paths in a CSV file.

Hello.

I have a CSV file that contains Source path and Destination Path info.  I wish to write a batch file that will use the CSV file to:
1) Create the Destination directory (if it doesn't exist)
2) Physically copy the Source file to the corresponding Destination directory.

Below is a sample of the first few lines of the CSV.

This needs to run on Windows Server 2003.
