To install the SBS transition pack during the installation of the SBS(i.e before i put clients on the CAL already pruchased) or after the 75 CAL is exhuasted?

You DON'T install the transition pack unless you have to. The transition pack means your SBS is no longer running as an SBS - and that will result in you losing a lot of functionality. So, only when you have reached the limits of SBS - such as the 75 user account limit - do you actually run the transition pack to remove the limits.

Also, don't forget that if you do ever reach the limits, there will be Essential Business Server (EBS) being released later this year, which allows you to have an environment similar to an SBS but with slightly more relaxed licensing requirements.
