Login page reloads with OWA 2003 through ISA 2006

I am attempting to setup Outlook Web Access 2003 through ISA Server 2006, I can access the login page over SSL fine, however on logging in the page reloads. If I type an incorrect username and/or password I get a login failed message - however a correct username and password just reloads the page back to the login screen.

Firewall Rules are attached, the web listener is configured as follows:
Networks: External, Internal (for testing purposes)
Connections: Enable SSL 443
Certificates: single domain name certificate selected
SSO: Enabled, sso domain .domain.com
Forms: Use customized disables, password management disabled
Authentication: HTML Form Authentication, Validation Method = Windows (AD)

I am testing it from a Virtual Machine with the hosts file changed to match the ISA server IP address rather than the OWA server address.

Have you disabled forms based authentication on the exchange server ? If not you need to do that.

goto exchange system manager>Servers>Expand>Protocols>HTTP>Right Click Virtual Server & Properties>Settings

Untick OWA enable FBA
