Cannot find 'Item' in the Insert drop down

Users are on Outlook 2003 SP3 connecting to a Exchange 2007 SP1 server.  One particular user cannot find 'Item' under the File/Menu Insert drop down.  

For the life of me I cannot find where this could be.

If MS Word is configured as the default mail editor then the Item object will be located on the PaperClip button drop down selection list.  If Outlook is configured as the default mail editor then the Item object will be in the Insert menu drop down list.
