PDF-Form not taking data in from XFDF

Hi Experts:

I have a XFDF File which is supposed to send Data to a PDF. Everything seems to be by the book. I am using Formfiller from http://koivi.com/fill-pdf-form-fields/ which does great in making XFDF files (see included snippet). And when I use Koivi's PDF to send the data to, it actually works.

So I made my own file and....the file opens when the XFDF is called, but no data gets transfered. So something must be wrong with the PDF (attached). Does anybody have an idea what is wrong with my PDF file (or XFDF for that matter)?
