As I had stated in the question, I had already downloaded the certificate to the phone.  It did not work.  However, this question can be closed as I was able to get this working finally.  The fix ended up being that I downloaded the iPhone Configuration Web Utility for PC's and created the profile from their.  For some reason when the account was created manually it would not work, but when created with the utility and imported to the phone it worked fine.  I looked at the settings and they were the same as far as I could see.

I'm not sure why this was the accepted solution, as I had said the question could be closed since I had found the solution for myself.  I had already tried the solution that lawrenceahawkins had presented.  The fix was to download the iPhone configuration tool and create the e-mail profile their.  The solution had nothing to do with Certificates.  The original question stated that I couldn't even connect to the Exchange server even when certificates were not required.
