Unable to remove TROJ_MEDPINCH.A virus

I have been trying to get through to Trend Micro Tech Support for the better part of a week with a problem that has our Antivirus Server down. I have been onb hold for hours ( one sitting was 3 1/2 hours on hold) and their tech support has not responded to any of my case submissions. So I'm hoping someone here can help me.

Our server (running Windows 2000 Server SP4) has been infected with the TROJ_MEDPINCH.A virus.

The solution Trend Micro outlines as the fix for TROJ_MEDPINCH.A does not work for a Windows 2000 Server.
Trend  Micro states:
Removing Autostart Entry from the Registry
This solution deletes the registry entry added by this spyware. Before performing the steps below, make sure you know how to back up the registry and how to restore it if a problem occurs. Refer to this Microsoft article for more information about modifying your computer's registry.
1.      Open Registry Editor. Click Start>Run, type REGEDIT, then press Enter. /li>
2.      In the left panel, double-click the following:
HKEY_LOCAL_MACHINE>SYSTEM>CurrentControlSet>
Services>SharedAccess>Parameters>FirewallPolicy>
StandardProfile>AuthorizedApplications>List
3.      In the right panel, locate and delete the entry or entries whose data value is the spyware path and file name of the file(s) detected earlier.
4.      Close Registry Editor.
5.      The problem is that my Windows 2000 Registry does not have FirewallPolicy>
StandardProfile>AuthorizedApplications>List under Parameters

Another problem: I downloaded the latest pattern file, lpt453.zip
However, when I unzip it, I dont see an executable. How do I apply these pattern files?
