creating new active directory account does not create exchange account
try to create them under the exchange's menu Active directory users.
Do you have the Exchange system manager installed on the server where you're creating the user's account? If not, the mail account information will not be created with the user.

exchange will not create the mailbox immediately - by design to reduce load if you create heaps of users
restart exchange, or force update of recipient update service/offline address list.
Sometime send an email to the user and exchange will then create the mailbox

OK found the issue.
It was in the RUS. I have a old exchange 2000 server that has been removed from the network for 5 months, but it is still listed in the Exchange system manager and for some reason the RUS was bound to that dead server.
So my next question is how do I remove that old server from the Exchange system manager so this does not happen again.
