Quick Launch Tool Bar Icon's display incorrectly

This is an issue I thought I've fixed a few time, only see it again.  So I figured I would post the question here to see if I can find a permanent solution.

Quick Launch Tool Bar Icon's display incorrectly (The Excel shortcut will actually have the Show Desktop icon or Internet Explorer shortcut will have the Excel Icon)

Attempted fixes:
I've tried every fix I've found online and some seemed to work for a week or so and then it would start up again
1) Updated Max Cached Icons registry key to 4096
2) Recreated the ShellIconCache FIle by toggling my color depth and changing the Icon size in Appearance settings
3) Deleted the IconCache.db

I know its as easy as toggling Quick Launch off and on to get the icons back to normal, but I'm really hoping to have something more permanent to give my end-users.  All of the Effected machines are Windows XP Pro, SP2 and have a base set of software (office suit, textpad, adobe reader, and winzip).  Hardware differs betweens affected machines.
