Uploading a file via HTML and PHP on to ftp server

Hello!

I'm going nuts trying to figure out why this doesn't work. Essentially, I am trying to build a REALLY simple website with just the browse button. User clicks "Browse", selects the file to upload to the server, and after clicking "Upload", the file should be uploaded to the file ftp server.

After researching online for code snippets (since this is the first day I've played with this), I keep getting an invalid file message.

Here are my code snippets. I've attached the 2 files to this message. Could someone tell me where I am going wrong here?....

What I do know are a few things about this ftp server:
> If I go to the ftp://.... , I need to enter a username and password for me to login and manipulate files. If I am uploading a file to a ftp://... server, does it matter if I add a username/password to the upload_file.php file??

 In the *.php file, does anyone know how to add 'error' messages so that after I click on upload, I can actually see what the error message IS?

Otherwise...can someone give me any alternative solutions?
