Follow up allerts not appearing

Hi all,
i have a user that sets her follow ups on her notes mail, but the follow up prompt never appears. We have refreshed the design, run a fix up, run a compress and reinstalled notes on her PC. Any ideas?

1) Check that alert notifications are turned on in the user preferences.  Turn it off and then on again , saving after each time.

2) Do a replace design rather than refresh if the above does not work.

Replace design has now been done, no luck. Awaiting user to return from a meeting to test alert notifications.
