Calendar entries dissapearing

I have an ongoing issues with outlook 2007
We have a number of PA's which are booking appointments for directors (they have full delegate access to their calendars).
We have been informed that randomly (i.e. not every time) a meeting request is sent out, and it appears that all recipients have confirmed attendance, but one of the directors doesnt have it in their calendar hence misses the meeting!!!
It doesnt seem to be happening to one director its happening across the board.
Does anyone have any ideas whats happening here??
