Attachments sometimes very very slow to open - no specific attachment type

Several users - OS = XP SP3 - Outllook 2003  SP3
All users are suffering from really slow opening of attachments - it doesn't happen all the time, one minute it is fine the next anything upto 45 secs to open a single page Word doc or similiar. All users are running cached mode from a 2003 SBS Dell server.
All virus / ad / malware SW is in place - I have tried disabling the virus protection to no avail still the same syptoms. All machines have recently been defraged.
Any ideas greatly appreciated

You can also try disabling the cache mode of Outlook:
1. Close Outlook.
2. Go to Start > Settings > Control Panel and double-click the Mail icon.
3. Click Email Accounts.
4. Select View or change existing e-mail accounts; click Next.
5. Select "Microsoft Exchange Server"; click Change.
6. Uncheck the "Use Cached Exchange Mode" box.
7. You will now be back at the Exchange Server Settings window. Click Next.
8. Click on Finish.
