Hi,

Is it possible to use bitlocker with TPM+USB+PIN and if so how?

Yes - but only works with certain versions of the OS and only certain machines. The how is down to the notes from the machine manufacturer. For example, HP and Toshiba laptops have different procedures in their bios regarding the setup.  We use Vista Enterprise but I believe Ultimate supports Bitlocker also.
