I think I know this, I had a very similar problem some time ago (although on Version 12.5). Same Error messages at least... you might wanna post a bit more details (exact error code and tell us what you were trying to backup / duplicate)

In my case, the problem occured if your job fits the following criteria:
- A duplicate of a Backup Set that was created using AOFO
- The job contains a MS SQL Backup, a MS Sharepoint or a MS Exchange Backup using the respective special agent.

I solved this by splitting my backup jobs. One is doing SQL, SPS and Exchange Backups only, and AOFO is disabled. The other is doing any Files / Shadow Copy / SystemState stuff with AOFO enabled. Both jobs are duplicating to tape.

So please provide us some additional information. in the meantime you might wanna try my idea.
