If you have 12.3 code (or late 12.2), you can insert lines using the sequence numbers. When you view the ACL with the "show access-list" command, each line will have a number in front of it (10, 20, 30, etc.). To insert a line between 20 and 30, just precede the command with a number between 20 and 30 (i.e. 25).

To delete a line:
conf t
ip access-list standard TEST
no permit 10.3.3.3 0.0.0.0
end

Router# show access-list TEST
Standard IP access list TEST

 10 permit 10.3.3.3 0.0.0.0
 20 permit 172.16.0.0 0.0.255.255
 30 deny 172.17.1.5 0.0.0.0
 40 permit 172.17.0.0 0.0.255.255

conf t
ip access-list standard TEST
35 deny 172.17.1.6 0.0.0.0
end
