Smart Host

We are using a Mail security gateway to scan emails going in and out; however if a person with an account in exchange 2007 is sending an email to another person in who has an account in the same server, it is not going through the smart host. Is it possible to send all internal and external traffic to first go through the smart host?

In this case, the mail is not going in, or out therefore why would you need to scan it? You already have desktop, server and mail server protection (I'd hope) and the MTA on the server will see that there is no need to send the mail on as it knows it has the mailbox.
