All Sites blocked by 2Wire Gateway's Internet Access Control

All requests through the gateway are being blocked by IAC (Internet Access Controls) on the router.  Requests appear in the Access Controls log and browsers are re-directed to:

http://gateway.2wire.net/access/site_blocked.html?NODEID=48&APP=IAC

This also happens for clients that are marked  in IAC "All Allowed".

Aditionally when typing in the admin password to temporariily allow access, access is still denied.

nslookup on a site returns correct IP's from the gateway.

A factory reset fixes the problem but not the cause.
