I have installed PHP on an IIS server.  Every time I try to browes to the test.php file I get a box to type in a username and password.

just as a test, I added "everyone" to both the web folder and the c:\php folder.

If I type in the admin username and password it will show the page.

what am I missing?

You need to add the IUSR_<XXXXX> account not 'everyone'
